The Human Centipede 4 is the hypothetical sequel to The Human Centipede 3 (Final Sequence), the final film in the Human Centipede trilogy. Although Human Centipede creator Tom Six has stated that The Human Centipede 4 will never happen, he has expressed an interest in making a spin-off featuring the Human Caterpillar.

As Tom Six developed the Human Centipede series, he envisioned a trilogy of films that acts as a "movie centipede", in which each installment can work as a standalone film, while being able to join together to create a single 4.5 hour film.[1][2][3] In 2014, Six spoke of the possibility of a fourth film, stating "Well, I planned it to be a trilogy. But never say never."[4] However, Six has since retracted the statement, saying that the third film is indeed the last.[5] According to Six, "I had always envisioned that I would make a trilogy of Human Centipede films. If you look closely, the films themselves are a centipede, and you can watch them back to back and the story flows from one film to the next. And I thought since [The Human Centipede 3 (Final Sequence)] is the last one, I wanted to go out with a bang. I had always envisioned the trilogy as a comedy. Human Centipede 3 is the kind of movie I wanted to make with the first one."[6]

When discussing the potential plot for a fourth Human Centipede film, Six joked that it would either feature a charity organisation connecting starving Africans in a human centipede in order to solve the hunger problem, or it would revolve around extraterrestrial beings connecting the entire human race into a centipede.[7]

Tom Six has expressed an interest in making a spin-off of the Human Centipede series starring the Human Caterpillar, a surgical construction that was introduced in Final Sequence. "After the trilogy [there will be] NO more Human Centipede movies, BUT I promise I'll make a Human Caterpillar in a few years cause I have a great idea for that," Six said on the matter. If made, the film will have a female lead and villain, and it will be shot in Japan.[8][9][10][11]
